Insurance

I accept Aetna, Anthem (BlueCross BlueShield), and United Healthcare (Optum).

If you plan to use insurance, your out-of-pocket cost will depend on your specific plan and benefits. Depending on your coverage, you may be responsible for a copay, coinsurance, deductible, or other cost-sharing amount.

I recommend contacting your insurance company before beginning therapy to confirm your behavioral health benefits and expected out-of-pocket costs.

Have questions about insurance or payment? Feel free to reach out before scheduling, and I’m happy to help you understand the billing process.

How to Check Your Insurance Benefits

Before your first appointment, I recommend confirming your mental health benefits directly with your insurance company. You can call the Member Services number on the back of your insurance card or check your plan's online member portal.

When contacting your insurance company, you may want to ask:

  • Do I have coverage for outpatient mental health services?

  • What is my copay or coinsurance for an individual therapy session?

  • Do I have a deductible that applies to mental health services? If so, how much of my deductible has been met?

  • What will my estimated responsibility be for an individual psychotherapy session?

  • Are telehealth and in-person therapy both covered?

  • Do I need a referral or prior authorization for therapy?

You can let them know that you are seeking outpatient individual psychotherapy with an in-network Licensed Professional Counselor (LPC).

Please keep in mind that information provided by your insurance company is an estimate of benefits and does not guarantee coverage or payment. Your final responsibility is determined by your insurance plan after your claim is processed.

You may be asked for the following information:

Provider: Abby Miranda, LPC

NPI: 1790607042

Service: Outpatient individual mental health therapy

CPT Codes: 90791 — Initial diagnostic evaluation

90837 — 60-minute individual therapy session

Out-of-Network & Private Pay

If I am not in-network with your insurance plan, you are still welcome to work with me as a private-pay client. Some insurance plans include out-of-network mental health benefits that may reimburse you for a portion of the cost of therapy.

If you'd like to use out-of-network benefits, consider asking your insurance company:

Does my plan include out-of-network benefits for outpatient mental health services?

Do I have an out-of-network deductible, and how much have I met?

What percentage of the cost will my plan reimburse after my deductible is met?

How do I submit a superbill for reimbursement?

For out-of-network clients, payment is due at the time of service, and I can provide a superbill that you may submit to your insurance company for possible reimbursement.

You may also choose to receive services on a private-pay basis without using insurance benefits.

Please keep in mind that benefit information provided by an insurance company is an estimate and does not guarantee coverage, reimbursement, or payment. Your insurance plan determines your benefits and financial responsibility.
